Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed, and how you can get access to this information.

Please review it carefully. It applies to DiscovHER Health LLC, the women’s specialty practice at 113 Oronoco St, Alexandria, Virginia.

Effective October 7, 2026. Questions: info@discovherhealth.com or (202) 394-3849.

We are required by law to maintain the privacy of protected health information, to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice that is currently in effect. Website browsing and HERshop orders are described separately in the Privacy Policy.

How we may use and share medical information

The law allows us to use and share your health information for treatment, payment, and health care operations without asking you again each time.

Treatment

We use your information to provide, coordinate, and manage your care. That includes sharing relevant details with another clinician who is treating you, a lab or imaging center, a pelvic-floor therapist we refer you to, or a pharmacy filling a prescription we send. Virtual visits use the same record as in-person visits.

Payment

DiscovHER is a self-pay practice for visits. We may share the minimum information needed to collect payment from you, to submit a claim you ask us to support through Reimbursify, to work with CareCredit if you choose financing, or to bill a program or treatment package you purchased. We may tell a health plan about a treatment you are going to receive when the plan asks, if you have asked us to help with reimbursement.

Health care operations

We use information to run the practice: appointment scheduling, quality review, training of our own staff, accreditation, auditing, and customer service. When we do this, we limit what is used to what the work requires.

Other uses that do not require your written authorization

  • People involved in your care. We may share information with a family member or friend who is involved in your care or payment, if you agree or do not object when you have the chance. In an emergency, we use our professional judgment.
  • Required by law. We share information when a statute, a court order, or a public-health authority requires it, including certain reports of abuse or a serious threat to health or safety.
  • Health oversight and public health. We may disclose information to a health oversight agency for audits and to public-health authorities for the purposes the law specifies.
  • Workers’ compensation and similar programs. We may disclose information as authorized by the laws that govern those programs.
  • Law enforcement and legal proceedings. We may disclose information in response to a valid court order, warrant, or the limited requests the privacy rule allows.

Uses that require your written authorization

Other uses and disclosures are made only with your written authorization. That includes most marketing that is paid for by a third party, and the sale of health information. We do not sell patient lists. You may revoke an authorization at any time in writing, except where we have already acted on it.

Appointment reminders and information about treatment alternatives or health-related services we offer may be sent to the phone or email you give us. Tell us if you want a different method.

Your rights

You have the following rights with respect to your health information. To use them, contact the practice at the address below. We may ask you to make the request in writing.

  • Inspect and copy. You may see or get a copy of the medical and billing records we use to make decisions about you. We may charge a reasonable, cost-based fee. We may deny the request in the limited cases the law allows, and you may ask for a review of some denials.
  • Amend. If you believe information is incorrect or incomplete, you may ask us to amend it. We may deny the request if we did not create the record, if it is not part of the records we keep, or if it is already accurate and complete. You may add a statement of disagreement.
  • An accounting of disclosures. You may ask for a list of certain disclosures we made in the six years before your request, other than disclosures for treatment, payment, health care operations, and the other exceptions in the privacy rule.
  • Request restrictions. You may ask us not to use or share information for treatment, payment, or operations. We are not required to agree, except that if you pay a service in full out of pocket and ask us not to share that service with your health plan, we will honor that request unless the law requires the disclosure.
  • Confidential communications. You may ask us to contact you at a specific phone number or address. We will accommodate reasonable requests.
  • A paper copy of this notice. You may ask for a paper copy at any time, even if you agreed to receive it electronically.
  • Breach notification. You will be notified if a breach of your unsecured health information occurs, as the law requires.

Our duties

We are required to maintain the privacy of your health information and to notify you of our legal duties. If the terms of this notice change, the new notice will be available at the clinic and on this page, and it will apply to all information we already have as well as information we receive in the future. We will not use or share your information other than as described here unless we change the notice and make the new notice available.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.
